Post-hospital care in Atlanta. The first weeks home, which are the riskiest.
Most readmissions happen in the first month, and usually for dull reasons: a missed medication, a meal skipped, a follow-up appointment nobody could drive to. This is cover for exactly that stretch, and plenty of families stop after a few weeks.

What a caregiver does
- Medication reminders on the discharge schedule
- Meals, and making sure they are eaten
- Moving safely around a house that suddenly has obstacles in it
- Rides to follow-up appointments, with someone who goes in too
- Telling you if something looks like it is going backwards
Who this suits
Anyone just discharged after surgery, a fall or an illness, especially if they live alone.
The riskiest four weeks
Readmissions cluster in the first month after discharge, and the reasons are usually dull rather than dramatic: a medication taken twice or not at all, meals skipped because shopping was impossible, a follow-up appointment missed because nobody could drive.
None of that needs a clinician. It needs somebody in the house who knows what the discharge summary said.
Where people here are discharged from
Our clients come home from Wellstar Kennestone in Marietta, Wellstar Cobb in Austell, Wellstar Paulding in Hiram, Wellstar Douglas in Douglasville and the Northside and Emory hospitals on the north side of Atlanta. Discharge planners at several of these know us.
If a discharge is being planned this week, call before it happens rather than after. Setting cover up in advance is far easier than arranging it at nine o'clock on the evening somebody gets home.
It is usually temporary, and that is fine
Plenty of families use this for two to four weeks and stop. Some keep a lighter schedule afterwards. Either is normal, and we will not put you in a long commitment to find out which.
The practical bits
- Minimum four hours per visit, with no minimum number of days a week.
- Care usually starts within 24 hours.
- A written plan with the hours, the tasks and the weekly cost before anything begins. See how it works.
- Nurse-owned, and licensed by the Georgia Department of Community Health as a Private Home Care Provider, permit PHCP010959.

Not sure what your parent actually needs?
That is the normal starting point. Describe the week and a care manager will tell you what would actually help, including if that is less than you think.

Post-Hospital Care: common questions
How fast can cover start after a discharge?
Usually within 24 hours of agreeing the plan, and faster if you call while the discharge is still being arranged. Tell us the discharge date on the first call.
Can a caregiver give the medications?
No. A caregiver reminds; they cannot administer. That line is set by our Georgia Private Home Care Provider permit, not by us. In practice reminders on the discharge schedule prevent most of the medication problems that cause readmissions.
Will you talk to the discharge planner?
Yes, with your consent. It is much easier to set the first week up correctly if we hear what the hospital actually recommended rather than reconstructing it from a folder.
